Robert Califf on Professionalism in Healthcare
On February 18, the UNC community came together for the 47th Merrimon Lecture, featuring distinguished speaker Robert Califf, MD, MACC. His talk, “Can We Balance Professionalism with the Extreme Financial Focus of U.S. Healthcare?”, encouraged attendees to reflect on the growing tension between clinical professionalism and the increasingly complex financial forces shaping American healthcare.
Dr. Califf explored how the financialization and fragmentation of the U.S. healthcare system have generated wealth for owners and intermediaries while contributing to moral injury among frontline clinicians and public health professionals. These pressures, he noted, also play a role in declining public trust and the rising influence of alternative health approaches—many fueled by widespread misinformation.
“We’re facing a moment when the balance between professionalism and financial incentives is out of alignment,” Dr. Califf explained. “Layer in the massive effects of digital communication, artificial intelligence, and misinformation, and we have a perfect storm—one that contributes to stagnant life expectancy in the U.S. and our lagging outcomes compared to other high-income countries. We must actively engage in public discourse, champion evidence, and uphold our professional responsibility to put patients’ health ahead of financial considerations. It’s not easy, but the stakes are high.”
The lecture sparked meaningful dialogue and left attendees with a renewed commitment to fostering an ethical, sustainable, and patient-centered healthcare system.
